If you're reaching out about midwifery care during your pregnancy, please include the following information:

- How far along you are and your estimated due date (if you know it)

- the date of the first day of your last menstrual period 

- A list of any labs or ultrasounds completed during your pregnancy (and if there were any abnormal results)

- What number pregnancy this is for you 

- How many previous pregnancies you have had, and any complications with your previous pregnancies. 

- If you have had a cesarean section 

- A little bit about you!

This gives me a starting point on your pregnancy, and a good idea of whether or not you'll be a good candidate for out of hospital birth! Thanks!
